Ultrasound report format or template- female abdomen


USG SCAN of the abdomen was done.

Liver:
Echogenicity: normal
Echotexture: normal
Mass lesions:nil
Portal vein: 11 mm diameter.

GALL BLADDER:
Shape and size: well distended.

SPLEEN:
Shape and size: normal
Mass:-- nil.

PANCREAS:
Normal appearance.

KIDNEYS:
LT.--  8.4 x 4.9 cms.       ; no pelvicalyceal dilation seen.
RT.--  8.5 x 4.5 cms.      ; no pelvicalyceal dilation seen.
Renal parenchymal thickness: Lt.-   1.7 cms         Rt.— 1.5 cms
Perirenal spaces are normal.
Renal cysts—nil.

PARAAORTIC REGIONS:
Any mass/ lymph nodes:-- nil.

 
URINARY BLADDER:
Distension— moderately distended.
Any trabeculation: -- no evidence of trabeculation seen.
Calculus:-- no evidence of calculus seen
Diverticulum:-- no diverticulum is present

UTERUS:
Size of uterus:
Endometrial thickness:
No evidence of uterine mass seen.
ovaries- size:
No evidence of ovarian mass or cysts seen.

OPINION:
USG scan suggests à
  • normal appearance of both kidneys.
  • The liver appears normal.
  • No evidence of uterine mass seen.

Portal vein: 11 mm. at porta.
 CBD and biliary tree: normal appearance.

GALL BLADDER:
Shape and size: poorly distended.
Walls—normal.

SPLEEN:
Shape and size: normal
Mass:-- nil

PANCREAS:
Normal appearance.

KIDNEYS:
Size: Lt--  Rt---
Both kidneys appear normal. No pelvicalyceal dilation is seen.
Renal parenchymal thickness: Lt.-   1.5 cms          Rt.—1.6 cms
Perirenal spaces are normal.
No evidence of renal cysts seen.


PARAAORTIC REGIONS:
Any mass/ lymph nodes:-- no mass or lymph nodes seen.

URINARY BLADDER:
Distension—normal
Any trabeculation: no evidence of trabeculation seen.
Calculus: no calculus is present
Diverticulum: no diverticulum is seen.


OPINION:
USG scan suggests Both kidneys appear normal.
There is no evidence of renal cysts.
Urinary bladder appears normal.
Liver shows no evidence of mass lesions.

More abnormal thyroid sonography templates

1) Multinodular goiter:
Same as normal thyroid sonography report with the added paragraphs as below:

Both lobes show multiple small and large, echogenic nodules and few cystic lesions  of varying sizes from 0.5 to 1 cms.
Mild augmentation of vascularity is present, especially around the nodules.
A large (macro) calcific lesions is present in right lobe.
Diffuse enlargement of thyroid is present.
Findings suggest Multinodular goiter.

2) For parathyroid adenoma:
A hypoechoic nodule (0.6 cms.) is seen posterior to the lower pole of right lobe.
? parathyroid adenoma.

3) Thyroglossal duct cyst:
There is a complex mass (1.0 x 0.9 x 0.5 cms.) in the midline in submandibular region.
It shows both solid and cystic areas also.
No calcification is present. The lesion is very vascular.
  • Findings suggest infected thyroglossal duct cyst.
4) Another report of thyroglossal duct cyst:
There is a complex mass (2.5 x 1.7 x 1.8 cms.) just to the left of the left of midline (at the level of thyroid cartilage).
It shows both (2 in number) solid, hyperechoic, rounded areas with a cystic area also seen.
No calcification is present. The lesion is poorly vascular.
Findings suggest thyroglossal duct cyst.
 
5) Hyperthyroidism:
Marked increase in vascularity of thyroid is present.

Ultrasound scan report formats- Thyroid:

There have been many and repeated requests by junior sonologists for templates for ultrasound imaging. I am starting off with a few templates of various thyroid sonography reports:
1) Normal thyroid:

USG SCAN and COLOR DOPPLER IMAGING OF THE THYROID :

Left lobe:
It measures:  0.9 x 2.7 x 3.2 cms in size. No cysts or nodules were seen.
Rt. Lobe and Isthmus:
Rt. Lobe measures 0.9 x 2.7 x 3.4 cms in size.
No cysts or nodules are seen.
Color Doppler showed normal vascularity of the thyroid.
The lobes and isthmus appear normal in shape.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
Normal shape and echogenicity of both lobes and isthmus of thyroid gland.
No mass or cystic lesion seen.

2) Colloid cyst of thyroid:
Left lobe:
It measures: 1.2 x 0.9 x 2.5 cms in size.
Rt. Lobe and Isthmus:
Rt. Lobe measures 1.5 x 2.0 x 3.0 cms in size.
The right lobe shows a large cyst of (1.2 x 0.8 cms.), occupying nearly the middle third of the right lobe.
The cyst shows no septae or echogenic foci ( calcific lesion). The cyst shows fluid content.
There is increased vascularity of both lobes of thyroid.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
The right lobe shows a large cyst of (1.2 x 0.8 cms.), occupying nearly the middle third of the right lobe.
The cyst shows no septae or echogenic foci ( calcific lesion). The cyst shows fluid content.
There is increased vascularity of both lobes of thyroid.
Findings suggest colloid cyst of right lobe of thyroid. D/d: hemorrhagic cyst

3) Adenomatous or colloid nodule of thyroid:
Left lobe:
It measures: 1.2 x 1.4 x 2.8 cms in size.
There is a hypoechoic nodule of 5 mm. in the left lobe of the thyroid.
It does not show calcification or increased vascularity.
Rt. Lobe and Isthmus:
Rt. Lobe measures 2.0 x 2.5 x 3.4 cms in size.
The right lobe shows a large, inhomogenous nodule of 2 x 2.5 cms.
Rim of vessels is present.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
The right lobe shows a large, inhomogenous nodule of 2 x 2.5 cms.
Rim of vessels is present.
There is a hypoechoic nodule of 5 mm. in the left lobe of the thyroid.
Findings suggest- adenomatous (colloid) nodules.

4) Hashimoto's thyroiditis:
Left lobe:
It measures: 1.2 x 1.5 x 2.7 cms in size. No cysts or nodules were seen.
Rt. Lobe and Isthmus:
Rt. Lobe measures 1.2 x 1.4 x 3.2 cms in size.
No cysts or nodules are seen.
The thyroid shows mild, diffuse loss of homogeneity.
Diffusely increased vascularity is present.
The lobes and isthmus appear normal in shape.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
The thyroid shows mild diffuse loss of homogeneity.
No mass or cystic lesion seen.
Findings suggest Hashimoto’s thyroiditis.

5) Hemorrhagic cyst of thyroid:
 
The cyst shows a few septa (2 in number) with 1 echogenic, poorly vascular nodule (0.5 cms.).
No calcific lesions are seen. The cyst shows fluid content with few echogenic particles within it.
Rt. Lobe and Isthmus:
Rt. Lobe measures 1.4 x 1.8 x 3.0 cms in size.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
The left lobe shows a large cyst of 2.1 x 3.5 cms., occupying nearly the entire left lobe.
The cyst shows a few septa (2 in number) with 1 echogenic, poorly vascular nodule (0.5 cms.).
No calcific lesions are seen. The cyst shows fluid content with few echogenic particles within it.
Findings suggest hemorrhagic cyst of left lobe of thyroid.

Normal renal arterial color doppler study

Right renal artery is visualized on color Doppler imaging: often part of the renal artery is visualized in one image. By angling the probe in various positions, it is possible to see part or whole of the renal artery. I find it more difficult to visualize the left renal artery than the right. But this can vary from patient to patient depending on viewing conditions, bowel gas and the acoustic window.




                                                                           
The right renal artery in spectral doppler tracing (above) shows normal waveform and the PSV (peak systolic velocity) is almost 60 cms./sec. at a doppler insonation angle of 42. The RI is= 0.6. The normal PSV of the renal artery is appx. 80 to 120 cms./second. It is considered abnormal if the PSV> 180 cms./second and renal artery stenosis is likely.


The segmental artery of the right kidney is studied in this spectral Doppler tracing (below). It shows an RI of 0.6 which is normal. Any RI of > 0.8 is highly suggestive of renal arterial stenosis.
















The left renal artery also shows similar findings.
















The spectral Doppler waveform of the left renal artery and left renal segmental arteries is also normal.
