The above ultrasound images show what is called a bulky uterus. This term is used when the uterus is larger than normal for the age of the patient. The common causes include -1) the post partum stage (after delivery/ childbirth), when the uterus is still large in size after delivery (the process of shrinking in size of the uterus to its normal size is called involution), 2) fibroids, 3) adenomyosis etc. These patients often complain of discomfort in the pelvic region and menstrual problems. Another cause of enlarged or bulky uterus in middle aged women is increase in the BMI (body mass index). Higher the BMI and obesity are associated with larger or bulkier uterus. See this link: Bulky uterus and obesity The ultrasound images above show a diffusely enlarged uterus with no evidence of masses (tumors) or other pathologies.
The above is an ultrasound video clip of a case of bulky uterus seen in sagittal section. Observe the increased size of the liver without any other pathology. This was a middle aged (52 year) old female patient with back pain. Bulky uterus is a common finding among middle aged menopausal women.
This middle aged lady with the bulky uterus (same case as above) also has a significant deposition of fat within the liver, resulting in moderate fatty change. Many middle aged Asian women present with both bulky uterus and fatty liver.

ReplyDeleteSmall kidney stone (renal calculus) can be treated by certain medicines which can dissolve the stone. Even small kidney stones can sometimes be painful. Bulky uterus is a common problem among middle aged women. Again for this consult a gynecologist who might prescribe medicines to correct the problem. Both the above problems dont appear serious enough for surgery. Please email me images of the ultrasound scan.

Hello anonymous, enlarged uterus is often the indicator or diseases such as adenomyosis. Do you have pain during menses? Do you have irregular periods? If you dont have any other complaints, then mildly bulky uterus may be ignored. Please consult a gynecologist also. If possible please email me images of your ultrasound study.

Rinky, I would like to see a more detailed ultrasound report of the uterus. Bulky uterus with polycystic ovaries can hinder the process of having a child. Also your pain may suggest adenomyosis or endomteriosis. I would advise you consult a gynecologist at the earliest.

Hello Klanto, sometimes a lump or mass may not be seen on ultrasound imaging because ultrasound cannot see through fat, gas or bone.
ReplyDeleteHence if you are troubled a lot by this doubt of a lump in your abdomen, you can undergo a CAT (or CT) scan study of the lower abdomen. This is a little expensive and has a very low exposure to xrays. If you want you can have an MRI scan of the abdomen... this is absolutely safe. Please meet a radiology center or radiologist near you.

It is all about fibroids of the uterus. It will help you understand. Fibroids and bulky uterus go hand in hand as fibroids usually make the uterus a little/ or more bulky depending on the size of the fibroid.
Yes fibroids can also be a cause for infertility as it is indeed the case with submucosal fibroids- this can interfere with the process of implantation and cause menstrual bleeding etc.

My mother is 50 years old. She recently had an endovaginal scan that showed anteverted utereus which is 79.1X 56.8X 59.8 mm in size. Myometrium shows normal echo pattern . No focal space occupying lesion was seen within myometrium. Endometrium echo was thickened. Endometrial thickness is 12.0 mms Right ovary weighs 32.8 X 14.3 mm and left ovary weighs 26.4 X 16.9 mms. No adnexal mass is seen in pouch of douglas. Sonological findings are suggestive of bulky uterus with thickened endometrium.

Hello, your mother has a common problem for her age. if she has no other complaints like uncontrolled menses or bleeding per vagina, and not much pain or such, this finding of bulky uterus is not very significant. Endovaginal scan is done by inserting the ultrasound probe into the vagina to view the structure of the uterus; and hence this type of scan is very accurate, as the ultrasound probe goes close to the uterus. The endometrial thickness (mild thickening) needs to be followed up over a period of time by repeating the scan once in 2 months.
DeleteShe should have plenty of fresh fruits and vegetables and may have a normal diet. Regular exercise (a walk or swim) is good to improve general health.

Hi, A small fibroid might decrease your chances of conceiving, but cant prevent it.
DeleteYou and your husband must undergo a complete infertility work up/ examination at an infertility center.. he should be examined by a urologist and you by a gynecologist. The complex cyst must be followed up and if large (more than 4 cms.) must be removed. Bulky uterus per se cannot cause infertility.

I would advice you to please consult a good physician and also if required to meet a gynaecologist.the only thing that concerns me is the possibility of pelvic inflammatory disease. As I have said before bulky uterus and fatty liver per se are not serious complaints. The images that you have sent are not very clear and I would like to have a detailed study of the endometrium by a transvaginal scan. You have not mentioned her age...as a woman approaches menopause she undergoes more irregularity in her menses. The presence of severe bleeding with clots is and can be very serious and has to be managed by a gynaecologist. If it does not respond to medical treatment then it has to be treated surgically including if necessary the removal of the uterus.

Hello doc. I am Ola from Nigeria. Sometimes in Dec, 2012, i went for an ultra sound and was told i had fibroid even though small in size and just about the size of my finger. I have been having irregular, heavy and longer menstrual periods. I also feel severe pain on the left side of my pelvic region. All attempts at getting pregnant have been futile. ReplyDeleteHi, the good news is that none of what you have said sounds serious! Also the fibroid can be removed surgically without harm to the uterus. I must know the exact size and location of the fibroid to be able to give a better answer.
DeleteAlso the bulky uterus is not a serious condition as I have said. But if associated with painful menses .. bulky uterus is a sign of adenomyosis.

She got her ultrasound which showed 2 things - fibroid in uterus and bulky uterus. Doctor said fibroid is not a concern. But for bulky uterus further investigation is needed. Hello, among the many diseases that can cause bulky uterus in a 60 yr old are:
Delete-cancer of the uterus (which is serious and needs immediate attention)
- pelvic congestion syndrome (associated with increased blood flow in the pelvis) due to varicosities or dilated pelvic blood vessels.

UTERUS is bulky approx 101mm x 54mm. ENDOMETRIAL ECHO is markedly thick (17mm). ENDOMETRIAL HYPERPLASIA to be considered. Myometrial echoes are homogeneous. Cervix reveals normal echotexture. Both adnexae are normal. Ovaries are normal in size with immature follicles. POD is free. dear doctor please suggest does she need to have surgery to remove uterus?? hello, I would suggest that your mother should undergo a D and C at the earliest. Please consult your gynaecologist. yes, endometrial hyperplasia can be a precursor of malignancy or cancer. You do not mention your mother's age. In any case a D and C with histopathological report is most imperative.
Deleteafter this, based on the pathological report, the removal of the uterus or hysterectomy may be needed.

ReplyDeleteI am 24 years old and had an abortion 3 months ago but since then i do not have normal cycles so i consulted a doctor and had a scan than gave me the following results. The uterus is bulk 9.45 by 4.50cm with some echogenic material indicative of rpocs measuring 5.14 by 4.92 by 4.01cm. The cervix islong and internal os closed. There is no evidence of fibroids or any other abnormalities.
Both ovaries are normal in size and appearance
There are no adnexal masses or cysts seen
No fluid is seen in the Pouch of Douglas. What does this all mean, should i be worried

ReplyDeleteExcessive bleeding Is a common phenomenon In middle aged women. A gynaecologist would analyse her symptoms and signs and prescribe the required medicines. If despite this, bleeding persists then if needed the uterus is surgically removed.

Usually, bleeding from the uterus would be irregular during the approach to menopause. After some time, bleeding would slowly decrease and then finally stop.
DeleteIn some cases, bleeding would increase and an ultrasound scan is crucial in finding any cause for it like for example a growth or polyp or fibroid of the uterus.
If still bleeding persists in spite of no growth or other abnormalities then surgery is needed to remove the uterus.

Hello Priya, please send me the images / pictures of your scan if possible. Most likely you have some form of enlarged uterus (bulky) + adenomyosis which can be treated by consulting a gynecologist. Also PCOD has good results with conservative treatment + diet/obesity control and regular exercise. PCOD and bulky uterus are very common in your age group. again consult a gynecologist.

DeleteHi Doctor, I need your advice.I am 33 years old. I had an ultrasound today , Everything is normal. Its shows "Cervix appears bulky in size" . All others looks good. Is this bulky cervix , critical? We were trying for baby last few months.
ReplyDeleteNot much of an issue. Though I would advise a follow up ultrasound. I feel if all other factors are OK then you should be able to have a baby.
