Ultrasound scan report formats- Thyroid:

There have been many and repeated requests by junior sonologists for templates for ultrasound imaging. I am starting off with a few templates of various thyroid sonography reports:
1) Normal thyroid:

USG SCAN and COLOR DOPPLER IMAGING OF THE THYROID :

Left lobe:
It measures:  0.9 x 2.7 x 3.2 cms in size. No cysts or nodules were seen.
Rt. Lobe and Isthmus:
Rt. Lobe measures 0.9 x 2.7 x 3.4 cms in size.
No cysts or nodules are seen.
Color Doppler showed normal vascularity of the thyroid.
The lobes and isthmus appear normal in shape.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
Normal shape and echogenicity of both lobes and isthmus of thyroid gland.
No mass or cystic lesion seen.

2) Colloid cyst of thyroid:
Left lobe:
It measures: 1.2 x 0.9 x 2.5 cms in size.
Rt. Lobe and Isthmus:
Rt. Lobe measures 1.5 x 2.0 x 3.0 cms in size.
The right lobe shows a large cyst of (1.2 x 0.8 cms.), occupying nearly the middle third of the right lobe.
The cyst shows no septae or echogenic foci ( calcific lesion). The cyst shows fluid content.
There is increased vascularity of both lobes of thyroid.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
The right lobe shows a large cyst of (1.2 x 0.8 cms.), occupying nearly the middle third of the right lobe.
The cyst shows no septae or echogenic foci ( calcific lesion). The cyst shows fluid content.
There is increased vascularity of both lobes of thyroid.
Findings suggest colloid cyst of right lobe of thyroid. D/d: hemorrhagic cyst

3) Adenomatous or colloid nodule of thyroid:
Left lobe:
It measures: 1.2 x 1.4 x 2.8 cms in size.
There is a hypoechoic nodule of 5 mm. in the left lobe of the thyroid.
It does not show calcification or increased vascularity.
Rt. Lobe and Isthmus:
Rt. Lobe measures 2.0 x 2.5 x 3.4 cms in size.
The right lobe shows a large, inhomogenous nodule of 2 x 2.5 cms.
Rim of vessels is present.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
The right lobe shows a large, inhomogenous nodule of 2 x 2.5 cms.
Rim of vessels is present.
There is a hypoechoic nodule of 5 mm. in the left lobe of the thyroid.
Findings suggest- adenomatous (colloid) nodules.

4) Hashimoto's thyroiditis:
Left lobe:
It measures: 1.2 x 1.5 x 2.7 cms in size. No cysts or nodules were seen.
Rt. Lobe and Isthmus:
Rt. Lobe measures 1.2 x 1.4 x 3.2 cms in size.
No cysts or nodules are seen.
The thyroid shows mild, diffuse loss of homogeneity.
Diffusely increased vascularity is present.
The lobes and isthmus appear normal in shape.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
The thyroid shows mild diffuse loss of homogeneity.
No mass or cystic lesion seen.
Findings suggest Hashimoto’s thyroiditis.

5) Hemorrhagic cyst of thyroid:
 
The cyst shows a few septa (2 in number) with 1 echogenic, poorly vascular nodule (0.5 cms.).
No calcific lesions are seen. The cyst shows fluid content with few echogenic particles within it.
Rt. Lobe and Isthmus:
Rt. Lobe measures 1.4 x 1.8 x 3.0 cms in size.
The trachea is central in position.
The common carotid artery and jugular veins are normal.

OPINION:
The left lobe shows a large cyst of 2.1 x 3.5 cms., occupying nearly the entire left lobe.
The cyst shows a few septa (2 in number) with 1 echogenic, poorly vascular nodule (0.5 cms.).
No calcific lesions are seen. The cyst shows fluid content with few echogenic particles within it.
Findings suggest hemorrhagic cyst of left lobe of thyroid.
